CMI Healthcare Services seeks a Homeless Case Manager


Responsible for outreach contacts with homeless mentally ill individuals or homeless individuals that present with co-occurring disorders. Provides advocacy for homeless consumers until discharged from the program Develops linkage plan with goals, review and update of linkage plan as needed Make referrals for housing, psychiatric evaluations, vocational services, Mobile Crisis Team, health care services, mental health center or mental health services or other services as deemed necessary Provides assistance in obtaining income support Conducts alcohol and drug screening for educational assessment and referral purposes Maintains all documentation and keeps records current to include 90 Day P.A.T.H. Program Plan, TennCare eligibility, progress notes & case closures Meets with P.A.T.H. Program Participants face to face by making shelter and treatment facility visits, as frequently as needed to foster stability and decline in program recidivism Conducts life skills training, as needed Provides assistance to link consumers to TennCare Partners Participates in area homeless events and community partner and contract-required meetings.
Requirements:
Reliable transportation Education: BA/BS in Social Work, Psychology, or other Behavioral Science disciplines. Knowledge of homeless outreach programs preferred and the ability to conduct alcohol and drug-education groups. Experience: Experience with working with severely persistently mentally ill (SPMI) individuals or individuals with co-occurring disorders. Ability to work with minimal supervision, while demonstrating integrity and representing the agency within the community. Knowledge of community resources.
