Passport Services with the Probate Court Clerk

The Probate Court Clerk’s Office will offer Passport services at Benjamin L. Hooks Central Library on the 3rd Thursday of each month (except June 2025).

By Appointment Only: Call the Probate Court Clerk’s Office at 901-222-3771 to schedule.

Steps to Prepare

1. Complete the DS-11 Application:
   • Arrive 15 minutes early if you need to fill out the form.
   • Applications available on-site or visit:
     • www.usps.com/passport
     • www.travel.state.gov
2. Bring Required Documents:
   • Proof of American Citizenship (plus a photocopy):
     • Certified birth certificate, Consular report of birth abroad, Naturalization certificate, Certificate of citizenship, or previous U.S. passport.
   • Valid Photo ID (plus photocopy of front and back):
     • Driver’s license, U.S. military ID, Government employee ID, or learner’s permit.
   • Passport Photo:
     • $12 per person (2″x2″ size).

Fees (Check or Money Order Only)

• Adults (16 and older):
  • Passport Book: $130 (routine) or $190 (expedited).
  • Passport Card: $30 (routine) or $90 (expedited).
• Minors (15 and younger):
  • Passport Book: $100 (routine) or $160 (expedited).
  • Passport Card: $15 (routine) or $75 (expedited).
• Execution Fee (All ages): $35 (Money order or cash only).
• Copies: $0.20 per page (Library fee).

Special Notes for Minors (Under 16):

If one parent is absent, a notarized Form DS-3053 is required from the absent parent.
